This is a summary of the assessment of the State Health System Innovation Plan’s (Final Design) strengths and areas for improvement. Overall, the timeline, project milestones and deliverables, and health care delivery and payment transformation models seem to be appropriate and well aligned with the SIM initiative and the FOA requirements.
This plan builds on many unique, cross-sector efforts already underway in the state and offers a relevant design to transform the health and wellbeing of the state’s population. Recognizing the huge effect of social determinants on the health and wellness of individuals and communities, our approach seeks to transform more than just the healthcare system.
