Los Lunas Community Program

The Los Lunas Community Program (LLCP) provides support and services through the Developmental Disabilities Waiver (DDW) to individuals who reside in Valencia, Socorro, Torrance and Bernalillo counties and provides technical assistance throughout the state.


Supported living, which allows persons with developmental disabilities and significant medical and/or behavioral challenges to live with roommates; supported employment, which helps participants find and maintain employment; and community membership and volunteerism opportunities, which give participants the chance to enhance interests and hobbies while developing friendships with other people in the community.

It is the mission of Los Lunas Community Program (LLCP) to promote a person-centered philosophy and provide ethical, effective, and efficient services and supports that assist those we serve to live the life they choose, establish meaningful relationships, foster opportunities for personal growth, and maintain healthy lifestyles.

Model of Care

LLCP serves adults eighteen years of age or older with intellectual and developmental disabilities (IDD).

LLCP provides the following services to persons eligible for the Developmental Disabilities Waiver DDW:

  • Supported Living
  • Intensive Medical Living Services
  • Customized Community Supports
  • Community Integrated Employment
  • Adult Nursing
  • Crisis Support

Supported Living, Adult Nursing, and Customized Community Supports are also provided with State General Funds, given their availability, to adults with IDD who have been referred by the NM Department of Health Developmental Disabilities Supports Division (DDSD).

LLCP also serves adults with IDD in our in our Intermediate Care Facility for persons with Intellectual Disabilities (ICF/IID), who have been court-ordered, and who have been assessed to be amenable to participation in our structured behavioral treatment program.

As a New Mexico Department of Health Facility, Los Lunas Community Program is a Home and Community Based Medicaid Waiver provider for New Mexicans with intellectual and developmental disabilities (IDD).

We offer a variety of services including Supported Living, Customized Community Supports, Community Integrated Employment, Nursing, and Crisis Services in Valencia County. We also operate an Intermediate Care Facility (ICF) for persons with IID who require a structured behavioral treatment program.

LLCP serves adults aged eighteen years of age and older, with DDW, SGF, or Medicaid funding.

Exclusionary Criteria

LLCP only serves adults eighteen years of age or older with intellectual and developmental disabilities (IDD) who indicate a need for our services and have appropriate funding from either the Developmental Disabilities Medicaid Waiver (DDW), State General Funds, or Medicaid.

Service Lines

LLCP is a New Mexico Department of Health facility, and a community provider of living supports for persons with intellectual and developmental disabilities (IDD). We provide the following services:

  • Supported Living – In the community of their choice, LLCP assists our clients to live as independently as possible providing supports designed to assist, encourage, and empower them to grow and develop, gain autonomy, become self-governing, and pursue personal interests and goals.
  • Intensive Medical Living Services (IMLS)- for those persons receiving Supported Living with acute medical needs, IMLS is provided to adequately meet their needs (e.g., more intensive staffing levels and levels of care)
  • Customized Community Supports (CCS) – Based on the preferences and choices of our clients, LLCP assists adults with IDD to increase their independence, lessen the need for paid supports, establish or strengthen interpersonal relationships, join social networks, and participate in community life.
  • Community Integrated Employment (CIE) – Based on the informed choice of our clients, LLCP assists adults with IDD to become employed in the community in jobs that increase their economic independence, self-reliance, social connections, and career development.
  • Adult Nursing: nursing services and medical case management is provided to our clients by a licensed nurse.
  • Crisis Supports: - short-term, intensive behavioral supports in a controlled environment for persons currently in crisis.
  • ICF/IID: LLCP operates an Intermediate Care Facility for persons with intellectual and developmental disabilities (ICF/IID) for persons who require a structured behavioral treatment program.

LLCP currently operates twenty-two group homes throughout Valencia County and serves 69 persons.
